We read with great appreciation the elegant response by Nardi et al.() to our comments() regarding their recent work() on multifocal papillary thyroid microcarcinomas (PTmC). We are particularly grateful for their collegial tone and thoughtful engagement with the points we raised concerning the prognostic landscape of this clinical entity. The discussion surrounding the clinical utility of total tumor diameter (TTD)(-) remains one of the most compelling debates in contemporary thyroidology. As the authors correctly pointed out, the existing literature lacks consensus, with divergent findings often arising from heterogeneous methodologies and differences in patient cohorts.
