Elderly patients in crisis: unveiling outcomes and management approaches in severe COVID-19 cases – a retrospective analysis from Brazil
DOI: 10.31744/einstein_journal/2025AO1428
Highlights ■ The elderly had higher severity, more comorbidities, and longer intensive care unit and hospital stays than adults. ■ They required more assistance resources and had higher hospital mortality. ■ Independent mortality predictors: age ≥60, frailty, arrhythmia, transplant, acute kidney injury, vasopressors and extracorporeal membrane oxygenation. ABSTRACT Objective: To analyze the sociodemographic and clinical characteristics, comorbidities, therapeutic resources, and mortality rates of elderly patients with COVID-19 admitted to an intensive care unit. Methods: This retrospective cohort study included patients […]

A pilot study on feasibility and hypothesis exploration: reducing on-scene length of stay of the emergency teams via ambulance dispatch teleconsultation for prehospital examination
DOI: 10.31744/einstein_journal/2025AO1469
Highlights ■ Teleconsultation reduced on-scene time from 36.8 to 20.6 min (p=0.019). ■ Video-based prearrival anamnesis improved team efficiency. ■ Interventions and clinical profiles were similar across the groups. ■ Study supports broader telemedicine adoption in emergency care. ABSTRACT Objective: Ambulance transport time is an important metric in prehospital care. Limited studies have explored strategies to decrease on-scene time. We examined the effect of collecting telemedicine-based medical data during ambulance dispatch on the on-scene evaluation time of the prehospital team. […]

Quality assessment of clinical practice in neuro-oncology
DOI: 10.31744/einstein_journal/2025AO1343
Highlights ■ The molecular test according to the WHO classification of CNS tumors (gliomas) was performed in 100% (172) of patients. ■ Education on chemotherapy and informed consent for patients with brain tumors was carried out for 100% (130) of the patients who had an indication for chemotherapy treatment. ■ Approximately 87% of the patients with gliomas underwent postoperative cranial MRI. ■ A multidisciplinary approach, including molecular testing, was used for 35% of patients with glioma. ■ Thromboembolic events after […]

Elevating care: assessing the impact of telemonitoring on diabetes management at a cutting-edge quaternary hospital
DOI: 10.31744/einstein_journal/2024AO0748
Highlights Telemonitoring reduced hospital readmissions in patients enrolled in the diabetes program by 15.4%. Men patients who received telemonitoring had lower readmission rates than those who did not. There was a difference in the readmission rate between those who underwent telemonitoring and those who did not in patients aged <60 and >75 years. Patients who stayed >10 days and underwent postdischarge telemonitoring had a lower readmission rate than those who did not undergo telemonitoring. ABSTRACT Objective: To assess whether post-discharge […]

Comparison of the effects of high-flow nasal cannula and bilevel positive airway pressure treatments as respiratory physiotherapy interventions for children with asthma exacerbation: a randomized clinical trial
DOI: 10.31744/einstein_journal/2024AO0588
Highlights Positive airway pressure may improve the severity of asthma exacerbation. A high-flow nasal cannula can improve respiratory physiotherapy outcomes. Positive airway pressure is indicated for moderate asthma exacerbation. Bilevel positive airway pressure and high-flow nasal cannula use may improve the pulmonary function. ABSTRACT Objective: To evaluate and compare the efficacy of high-flow nasal cannula treatment and that of bilevel positive airway pressure treatment as respiratory physiotherapy interventions for pediatric patients who are hospitalized because of asthma exacerbation. Methods: During […]

Inhaled nitric oxide in moderate-to-severe COVID-19 acute respiratory distress syndrome: a retrospective cohort study
DOI: 10.31744/einstein_journal/2024AO0578
Highlights Acute respiratory distress syndrome is a severe presentation of COVID-19 without effective treatment despite intensive research. Inhaled nitric oxide is a vasodilator that has been studied in patients with acute respiratory distress syndrome COVID-19 yielding contradictory results. The use of inhaled nitric oxide rescue therapy in critically ill patients with COVID-19 experiencing moderate-to-severe acute respiratory distress syndrome was significantly associated with an improvement in oxygenation parameters with no difference in mortality. ABSTRACT Objective: To evaluate the responsiveness of COVID-19 […]

Mortality, hospitalizations, and persistence of symptoms in the outpatient setting of the first COVID-19 wave in Brazil: results of SARS-Brazil cohort study
DOI: 10.31744/einstein_journal/2024AO0652
Highlights Previous influenza vaccination was associated with a reduced need for hospitalization after COVID-19 diagnosis. In 60 days of outpatient setting, four deaths occurred (three in the hospitalized and one in the non-hospitalized group). The persistence of more than two symptoms in 60 days was observed in 31.9% of patients. ABSTRACT Objective: To evaluate deaths, hospitalizations, and persistence of symptoms in patients with COVID-19 after infection in an outpatient setting during the first COVID-19 wave in Brazil. Methods: This prospective […]

Cost and time of hospitalization for elderly people with bone fractures in a reference hospital
DOI: 10.31744/einstein_journal/2024GS0493
Highlights Bone fractures were more frequent in women. The mortality rate among patients was 9.6%. The main trauma mechanism was falling from standing height. The total hospitalization cost ranged from R$2,006.53 to R$106,912.74. The mean daily cost was R$4,478.64. The longer the hospital stay for treatment, the higher the total cost. ABSTRACT Objective: To describe and analyze the aspects regarding the cost and length of stay for elderly patients with bone fractures in a tertiary reference hospital. Methods: A cross-sectional […]

Methods A retrospective, observational study. The medical records of patients included in the hospital Pediatric Sepsis Protocol and patients with discharge ICD-10 A41.9 (sepsis, unspecified), R57 (shock) and A39 (meningococcal meningitis) were evaluated. Results A total of 399 patients were included. The prevalence of sepsis, severe sepsis, and septic shock at the emergency […]
