COVID-19 and coinfection with Clostridioides ( Clostridium ) difficile in an infant with gastrointestinal manifestation
DOI: 10.31744/einstein_journal/2020RC6048
ABSTRACT We report the clinical case of an infant with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection with gastrointestinal signs and symptoms, predominantly vomiting. The patient also had colic, poor feeding, mild diarrhea and mild rhinorrhea without fever. The child had evidence of altered coagulation, increased interleukin 10, moderate dehydration and she was admitted to the pediatric intensive care unit. Simultaneously, the patient was diagnosed as Clostridioides difficile infection, which possibly may have facilitated the persistence of SARS-CoV-2 in […]

L-methylfolate, a new option in psychiatric treatment, would it be linked to psoriasis relapse?
DOI: 10.31744/einstein_journal/2020RC5522
ABSTRACT We report a case of a 61-years-old woman in remission of psoriasis for 20 years. She presented recurrence of psoriasis in the form of plaques few days after taking L-methylfolate 15mg/day. The L-methylfolate was prescribed as an adjuvant for the treatment of depression in a patient with the methylenetetrahydrofolate reductase gene polymorphism (MTHFR).

Vomiting in newborns as a result of a duodenal membrane: two case reports
DOI: 10.31744/einstein_journal/2020RC4641
ABSTRACT Vomiting episodes in newborns are extremely common and often attributed to gastroesophageal reflux. The symptoms of vomiting, however, may be caused by other complications. In this report, we present two cases of a 1-month-old male and a 2-month-old female, both presenting vomiting episodes that led to malnutrition. Some pediatricians often attribute the diagnosis of gastroesophageal reflux to newborns that are vomiting; however, there is a portion of the population that has other causes that lead to similar symptoms. The […]

Anaphylaxis to Agaricus bisporus ingestion
DOI: 10.31744/einstein_journal/2020RC5478
ABSTRACT A 33-year-old male with house dust mite allergic rhinitis and asthma reported an episode of facial and lip angioedema, dyspnea, cough and dysphagia at the age of 25, minutes after eating a mushroom ( Agaricus bisporus ) pizza. He denied any drug intake, hymenoptera stings or other possible triggers, and no identifiable cofactors were present. Since then he avoided all types of mushrooms, however an accidental contact occurred with mushroom sauce that resulted in angioedema of the lip within […]

COVID-19 on resonance magnetic: an incidental but important finding in times of pandemic
DOI: 10.31744/einstein_journal/2020AI5891
A 34-year-old woman with a personal history of ulcerative rectocolitis for 5 years who were using sulphasalazine and mesalazine. The patient was admitted to the emergency unit presenting asthenia, dyspnea, fever, cough with hemoptoic sputum for 8 days, and oxygen saturating by 97% in room air. The computed tomography (CT) () showed sparse ground-glass opacities, although more evident in the posterior contour of lower lobes. She was hospitalized and tested positive for coronavirus infection identified through real-time reverse-transcription-polymerase-chain-reaction (RT-PCR) test. […]

Squamous cell carcinoma implantation in gastrostomy orifice. Case report
DOI: 10.31744/einstein_journal/2020RC5409
ABSTRACT Percutaneous endoscopic gastrostomy is used to provide enteral nutritional support for patients with obstructive oropharyngeal or esophageal neoplasms. The placement of the catheter is considered safe, with few complications. Despite this, a specific complication that is considered rare, has been increasingly described in the literature, i.e., metastasis of head and neck cancer in the gastrostomy stoma. In this report, we described a case of metastasis of squamous cell carcinoma of the larynx in the gastrostomy site, and discussed the […]

COVID-19 myocarditis: a case report
DOI: 10.31744/einstein_journal/2020RC5876
ABSTRACT A male patient with flu-like symptoms and tomography and laboratory diagnosis of severe acute respiratory syndrome. He developed acute cardiac dysfunction during admission and was submitted to a cardiac magnetic resonance imaging examination, which confirmed acute myocarditis, indicating cardiac involvement by coronavirus disease 2019. A review and discussion about coronavirus disease 2019-related cardiac manifestations are reported, focusing on the imaging findings to make diagnosis.

Abdominal symptoms as initial manifestation of COVID-19: a case series
DOI: 10.31744/einstein_journal/2020RC5831
ABSTRACT The COVID-19 became a pandemic in early 2020. It was found, at first, that the main manifestations of this new virus occur through respiratory and constitutional symptoms. Therefore, chest tomography was elected as the best imaging test to assess the extent of pulmonary involvement and as a good prognostic predictor for the disease. However, as new studies were produced, the gastrointestinal involvement of COVID-19 becomes more evident, with reports from patients who manifested mainly or only gastrointestinal symptoms in […]

Autism associated with 12q (12q24.31-q24.33) deletion: further report of an exceedingly rare disorder
DOI: 10.31744/einstein_journal/2020RC5335
ABSTRACT Chromosomal abnormalities are responsible for several congenital malformations in the world, some of these are associated to telomeric/subtelomeric deletions. The abnormalities involving the telomere of chromosome 12 are rare, with few reports of deletions involving 12q24.31 region in the literature, and, to our knowledge, only four of them in the 12q24.31-q24.33 region. We report a further case of interstitial deletion of bands 12q24.31-q24.33 associated with autism spectrum disorder. A 2-year-old boy with global developmental delay associated with multiple congenital […]

The halo sign as a chest computed tomography finding of COVID-19
DOI: 10.31744/einstein_journal/2020AI5742
A 36-year-old male patient from New Jersey, USA, with obesity, hypertension and diabetes, was admitted to our service presenting excessive sweating, tachypnea, tachycardia, fever (38.6°C), and blood oxygen level of 90%. The patient reported that 5 days before he had sought medical care in the United States for dry cough, nasal congestion, malaise and fever. Chest radiography () and computed tomography (CT) () at admission showed multiple small nodular opacities and diffuse consolidations in both lungs, with central and peripheral […]
